In these cases, evaluations often result in the following diagnoses: tight iliopsoas, tight IT band, etc. While I agree that these conditions are present, I disagree with them as sole diagnoses and the recommendations often given to treat them. In cases like this young athlete, when the iliopsoas tendon or iliotibial band is connecting with the hip bone and making a grinding sound, I agree that that is part of the problem. But, recommending chronic stretching/massaging the tendon or surgically elongating it is not going to cure the problem. Why? Because it isnt the central problem, it is only secondary to the underlying condition. The underlying, central problem is joint instability due to ligament laxity. As the person travels from doctor to doctor, massage therapist to physical therapist to neurologist to kinesiotherapist to orthopedic surgeon, etc., what is happening to the joint instability? It is worsening. After potentially undergoing multiple cortisone injections, which further damage the soft tissues, often tendon tears and tendinopathy are eventually found on MRI that may convince the patient and the orthopedic surgeon that surgery is necessary. But, is doing surgery on gluteus medius tendon tear going to have long-term success at getting the patient back to full exercise if the underlying problem is joint instability? No it wont.

How Does It Feel

Snapping hip syndrome causes a snapping sensation and sound that can be felt in the front, the side, or the back of the hip. Often, the snapping can be pain free. If it causes pain, the pain usually ceases when the leg movement causing the snapping is stopped. The sensation is often experienced when an individual is required to use their hip to change positions. In athletes and dancers, the snapping can be accompanied by weakness and may diminish performance.

The snapping is most commonly felt when kicking the leg forward or to the side, when bringing the leg behind the body, when rising from a chair, or when rotating the body or the leg.

Often, walking and running in a straight line are snap free and pain free, although in some people, these activities are limited by the pain of the structure that is snapping.

If it is caused by tight muscles snapping over bony bits .. while its not that bad or necessary painful, it can lead to inflammation, bursitis and other problems at the hip in some people

    The people who get snapping hip syndrome the most are often dancers, athletes and people doing a lot of exercise or people that are using their hip flexors a lot in some way.

    How Can You Prevent Snapping Hip Syndrome

    • Warm up before starting a sport or heavy physical activity. Your warm-up should include stretches taught to you by your physical therapist, including those for the muscles on the front, side, and back of the hip.
    • Gradually increase the intensity of an activity or sport. Avoid pushing too hard, too fast, too soon.
    • Follow a consistent strength and flexibility exercise program to maintain good physical conditioning, even in a sports off-season.
    • Wear shoes that are in good condition and fit well.
    • Modify activity when symptomatic snapping occurs. Call a physical therapist.

    Causes Of Snapping Or Popping Hip Syndrome

Often, patients complain of a catching sensation within the hip joint in which the hip joint gets stuck and they have to jiggle the hip to get the hip to move normally again. These are signs that the person has intra-articular pathology. Both internal and intra-articular snapping hip can be confirmed by dynamic ultrasonography. However, self-reported snapping hip can usually be considered diagnostically sound.
